The kinds of things that used to wait for the next visit, because there was nowhere else for them to go.
The Care Journal is somewhere they can go, in your own words, in your own language, on your own schedule.
What you capture becomes part of how your care team sees your treatment, and part of how your trial learns from the people who lived it.

PRO-CTCAE is the patient side of the symptom framework that oncology trials use to keep track of side effects. The clinician side, CTCAE, is what your oncologist and your coordinator use to grade what they observe. The patient side, PRO-CTCAE, is what you observe.
A trial that hears both sides has a fuller record of the treatment. That fuller record is part of how cancer medicines get safer over time, for the people who come after you.
The Care Journal supports PRO-CTCAE for the symptoms most relevant to your treatment regimen. Your coordinator can explain which questions your specific trial uses.
Treatment is exhausting. Many patients ask a family member or caregiver to help with the Care Journal, especially on the days when even speaking takes effort. The Care Journal supports this two ways.
Share the linkA patient can share their Care Journal link with a family member who captures on their behalf. Every note is labeled clearly: “captured by [name] on behalf of [patient].”
The simplest form of help, no setup required beyond forwarding the link.
Appoint a primary caregiver accountA patient can appoint a primary caregiver account with their coordinator's help, so caregiver access is more durable across the length of the trial. The care team approves any delegation.
The Care Journal does not grant caregiver access without explicit patient and care team consent.
The family is part of the care team. The Care Journal is built to honor that.
Languages are added by request, with translators who specialize in clinical care. For accessibility issues, email accessibility@burna.ai. Accessibility bugs are fixed inside two weeks by policy.

Screen reader tested on NVDA, VoiceOver, and JAWS. Full keyboard navigation. High-contrast and dark modes. Font sizes adjustable to 200% without layout break. No timeouts on symptom entry; work is auto-saved. Available on any phone made after 2018.
A rash. A tremor. The way you walk when your feet are numb from neuropathy. Swelling that looks different in person than the words for it suggest. CareVid is the optional video companion to the Care Journal, launching in pilot during 2026. A short video of the symptom is recorded on a phone and sent securely to the care team. The team sees what you see.
CareVid is opt-in. You decide when a video is the right way to share something, and when words are enough on their own.
CareVid launches in pilot with selected US cancer centers in the 2026 design partner cohort. General availability planned for 2027. Coordinators can request earlier access for trials where it would help.
Your Care Journal entries are linked to your clinical trial record and visible to your care team only. Burna the company does not see your name, your address, your diagnosis, or your phone number. The full Privacy Policy is at /privacy.
Stored inside your hospital's systems or your region's healthcare cloud. They do not cross your country's borders without your consent. Burna processes your data on behalf of your care team, only for the purpose of supporting your trial.
A copy of everything Burna holds about you is available at any time. Anything that is wrong can be corrected. Consent can be withdrawn, with records removed from the Burna side of the platform.
GDPR rights apply in full in Europe and the UK. CCPA rights apply in California. In every other jurisdiction, the strongest applicable protections apply. The Care Journal does not show advertising, does not track you across the web, and does not sell your data to anyone, ever.

The Care Journal is structured to support the workflows associated with the CMS Remote Therapeutic Monitoring code family (CPT 98975, 98976, 98977, 98980, 98981, and adjacent codes within the 98975 to 98981 range). Patient-reported symptom capture, asynchronous transmission to the care team, and clinician review time are the workflow building blocks RTM was designed to recognize.
What the Care Journal contributes to an RTM-eligible program.
What the Care Journal does not do: the Care Journal does not bill on your behalf, does not determine code eligibility for your specific patient population, and does not replace your institution's revenue cycle review. Code eligibility, time thresholds, payer-specific coverage, and documentation requirements vary by program. Burna does not provide billing or coding guidance.

Ten questions we hear most often.
No. The trial team may use it to make their work faster and more complete, but capturing through the Care Journal is usually optional. Your coordinator can explain how it fits into your specific trial.
Then do not. The trial team has other ways to receive symptom information. The Care Journal never replaces human contact. It is one of several ways to reach your team between visits.
Your trial care team (your coordinator, your trial nurse, your physician) and no one else by default. Your hospital's standard patient privacy rules apply.
Edit it before you send. After it is sent, capture it again with a correction. Everything is versioned. The care team sees the full history, so “I meant to say fatigue, not nausea” is handled the same way it would be in a phone call.
Call your trial's 24/7 line, your doctor's office, or your local emergency number. The Care Journal is not an emergency tool. It is a way for your voice to be part of your care between visits.

The Care Journal shows you the transcription before you send. You can edit any word. The transcription system is trained on oncology terminology rather than generic dictation, so words like neuropathy or mucositis are recognized cleanly. If something comes out wrong, fix it; the system also learns from corrections over time.
A designated caregiver can access the Care Journal from anywhere their internet allows, with the patient's and care team's consent. The data still stays inside the patient's country of care.
No. Burna provides a patient-facing tool aligned with the workflows RTM was designed to recognize, and exportable records of patient transmissions and clinician review time. Code selection, eligibility determination, and billing are the responsibility of your institution's revenue cycle and compliance teams.
